Sam Holland was a railroad supervisor, watching the motorcade with a couple of his men on the triple overpass. When the shots started he saw a puff of smoke from behind the picket fence and as best he could tell the sound came from there as well.

Had the normal security protocols been in place, no one would have been on that overpass... but they were not.
